Forms & Surveys

The Forms & Surveys tab controls which forms and surveys are attached to this program, and how each form behaves in the Wayfinder.

Each attached form is a row with a Wayfinder toggle:

  • Wayfinder off — submissions are recorded and marked completed; nothing appears in the Wayfinder.
  • Wayfinder on — each submission creates a trackable item in the Wayfinder that moves through the program’s status workflow. Two settings appear:
    • Initial Status — the status each submission starts in (see below).
    • Side Peek Display and Export Fields — which of the form’s fields show in the Wayfinder side peek and which are included in CSV exports.

Initial Status — where a submission starts

The Initial Status dropdown picks the program status a new submission is born into. What that status means — whether it demands approval, records budget, or closes the item — is defined by its system-status mapping on the Statuses tab:

  • If the chosen status maps to Pending, every submission of this form lands in the Wayfinder’s Requires Action asking an Approver or Manager to approve or reject it.
  • If it maps to In Progress (or another non-Pending status), submissions skip the approval step entirely.
  • If no Initial Status is chosen, submissions default to Pending — this is the usual reason a form unexpectedly demands approval.

Statuses marked ⚡ in the dropdown record against the program budget and limits the moment a submission lands in them — you would rarely want an ⚡ status as an Initial Status.

Surveys attach alongside forms with their own label and an optional sessions label for multi-session use.
